Why continuous monitoring changes Parkinson's outcomes

A clinical exam every few months is one snapshot.
Daily data is the story between those visits.

1M+
Americans living with Parkinson's disease
Most seen by a neurologist every 3–6 months. Between visits, medication timing, on/off periods, and gait changes go unobserved - until a fall or crisis brings them in sooner.
Timing
Medication timing is the most critical daily variable
Parkinson's medications have a narrow therapeutic window. Late or missed doses directly cause on/off periods - the freezing, stiffness, and loss of control that most affect quality of life.
Gait
Walking speed & asymmetry — objective progression signals
Apple Watch provides walking speed, step length, and asymmetry data automatically. These metrics decline before symptoms become clinically apparent - giving you early warning of motor progression.
RTM
Billing eligible — no second condition required
RTM does not require 2+ chronic conditions. Parkinson's alone qualifies. Daily medication routines and wearable sync generate the data transmissions needed for billing. New 2026 codes cover 2–15 day engagement.
Simple enrolment

From referral to monitored patient
in under a week.

Medication adherence tracking and on/off period logging start immediately - no wearable required. Connecting Apple Watch or Oura adds gait metrics, HRV, and sleep data automatically.

1
Refer and set medication schedule
Set medication names, dose timing, and frequency. Isabella builds reminders and adherence tracking from your parameters. Under 3 minutes.
2
Patient onboards via Ziva app
Isabella walks the patient (and carer if involved) through their daily routine - medication logging, symptom tags, and wearable connection. Designed for ease with large tap targets and simplified interaction.
3
Gait and mobility data streams in
Walking speed, step length, step count, and walking asymmetry from Apple Watch populate the panel daily - automatic, no patient logging required.
4
Symptom tags logged by patient or carer
On/off periods, dyskinesia episodes, freezing, falls, and mood changes tagged in real time - or by a family carer if the patient has motor difficulty. Timestamped and correlated with medication timing automatically.
5
Monthly RTM report compiled automatically
Medication adherence log, on/off period frequency, gait trend, HRV, sleep, and Isabella's clinical summary - ready in the portal and EHR before every visit. Supports RTM billing documentation.
What Isabella tracks for Parkinson's patients
💊 Medication timing & adherence
Every dose timestamped against the prescribed schedule. Late and missed doses flagged. 30-day adherence percentage visible to provider and patient. The most impactful signal for managing on/off periods.
🏷 On/off period tags
Patient or carer tags on/off periods, dyskinesia, freezing, and falls as they happen. Timestamped and correlated with medication timing - making the pattern visible over weeks.
⌚ Gait metrics · Apple Watch
Walking speed, step length, and walking asymmetry - streamed automatically from Apple Watch. Objective gait progression data your clinical exam captures only at each visit.
😴 HRV & sleep quality
Autonomic dysfunction is a core non-motor feature of Parkinson's. HRV decline and sleep fragmentation tracked automatically - correlating with disease burden and symptom fluctuation.

Isabella · Clinical summary
"Robert's on/off periods have increased from 3 to 9 per week over 4 weeks. Medication adherence is 58% — the 6 late evening doses correlate directly with morning on/off episodes the following day. Walking speed has declined from 1.10 to 0.82 m/s and HRV is at 31ms. Two falls logged this month. Recommend reviewing medication timing schedule at next visit."
Clinical dashboard

See medication timing, on/off periods,
and gait - together.

A clinical exam every few months captures a snapshot of your patient's motor function at one moment. Ziva gives you the continuous picture - medication adherence correlated with on/off period frequency, gait trends from Apple Watch, HRV and sleep - all layered so the relationship between timing and symptoms is visible.

✓Medication timing log — every dose timestamped, late and missed doses flagged, 30-day adherence visible
✓On/off period tags correlated with medication timing — the relationship between late doses and symptom episodes made visible
✓Walking speed, step length, and asymmetry from Apple Watch — objective gait progression data between clinical exams
✓Fall and freezing episode logs — timestamped by patient or carer, with medication timing context
✓HRV and sleep quality — autonomic and non-motor symptom monitoring across your full panel
✓Carer access — family member or carer can log tags and assist with daily routine on behalf of the patient

Monthly reports & workflow

The full picture arrives
before every appointment.

Isabella compiles a monthly report covering medication adherence, on/off period frequency, gait trend, HRV, sleep, fall and freezing logs - auto-attached to your EHR before each visit. Your appointment becomes a clinical decision, not a history-taking session. The report also supports RTM billing documentation.

✓Medication adherence log — every dose with timestamp, late doses flagged, 30-day adherence percentage
✓On/off period frequency — weekly counts, correlation with late doses highlighted
✓Gait summary — walking speed, step length, and asymmetry trend over 30 days
✓Fall and freezing log — dates, times, and medication context for each event
✓RTM documentation — routine completion log with day counts supporting 98977 (16+ days) or 98985 (2–15 days)

"Worsening on/off period frequency (3 to 9 per week) directly correlates with declining medication adherence — 6 late evening doses each week this month. Walking speed down from 1.10 to 0.82 m/s over 4 weeks. Two falls logged. Recommend reviewing medication timing schedule and fall risk at next visit. 22 routine days completed — RTM 98977 eligible this month."
RTM billing

Parkinson's qualifies for RTM.
No second condition required.

Remote Therapeutic Monitoring (RTM) does not require two or more chronic conditions. Parkinson's disease alone qualifies. Ziva's daily routines - medication logging, symptom tags, and wellness check-ins - generate the data transmissions required for billing. New 2026 codes mean even partial engagement (2–15 days) is now billable.

Full engagement — 16–30 days / month
98975Initial setup & patient education (once)~$20 once
98977Device supply · 16–30 days/month~$40/mo
98980Treatment management · first 20 min~$50/mo
Partial engagement — 2–15 days / month (new 2026)
98985Device supply · 2–15 days/month~$40/mo
98979Treatment management · 10–19 min~$25/mo
Add-on
98981Treatment management · each additional 20 min~$45/mo
Ziva's daily routines and check-ins constitute the data transmissions for RTM device supply codes. Review eligibility and documentation requirements with your billing team. Approximate Medicare rates - many Parkinson's patients are on Medicare. 98985 and 98977 are mutually exclusive per billing period.

What drives RTM day count
💊
Daily medication log
Each logged dose is a timestamped data transmission - the most consistent daily signal for Parkinson's patients
🏷
Symptom tags & wellness logs
On/off tags, mood logs, and wellness check-ins each count as a transmission day toward the billing threshold
⌚
Daily wearable sync
Each day gait metrics, HRV, and sleep data syncs from Apple Watch - counts as a passive data transmission day
Illustrative only. Approximate Medicare rates - most Parkinson's patients are on Medicare or Medicare Advantage. Consult your billing team before go-live.
