Homocysteine

An amino acid that rises when B12, folate or B6 run low - and is linked with higher vascular risk.
TypeBlood test
HowFasting blood draw.
How oftenOccasionally, or to check B-vitamin status.
BeforeFast 8–12 hours.
WhoPeople investigating B-vitamin status or with strong vascular family history.

A window into B-vitamin function

Homocysteine is cleared by B12, folate and B6. When they run short, it builds up. High levels are linked to heart and brain risk - but lowering it with vitamins hasn’t clearly reduced heart attacks.
Raised
“Why is it high?”
Often low B12 or folate, reduced kidney function or genetics.
Find the cause
B-vitamins
“Will supplements help?”
They lower the number - worthwhile if you’re deficient.
Correct
Heart risk
“Should I test it for my heart?”
ApoB and Lp(a) are stronger guides.
Other tests first

Is it right for you?

Worth considering if…

  • You want a functional check on B-vitamins.
  • You have early vascular disease in the family.
  • Your B12 or folate is low or borderline.

A better option if…

  • You want your main heart-risk markers - ApoB and Lp(a) come first.

How it’s measured

Lab blood draw
Usually fasting.

The full picture

What is it?
An amino acid made during normal metabolism.
What type of test is it?
A blood biomarker.
What does it measure?
Homocysteine concentration.
Why might someone consider it?
High levels suggest low B-vitamins and are linked to vascular risk.
Who is it most relevant for?
People investigating B-vitamin status.
How is it performed?
A fasting blood draw.
How often might it be considered?
Occasionally.
What can the result help you understand?
Whether B-vitamin status may need attention.
What are its limitations?
Lowering it with vitamins hasn’t clearly reduced heart attacks in trials.
What should it be viewed with?
B12, folate, MMA, kidney function and ApoB.

Where to get it

Through your provider
The simplest route - often covered when there’s a clear reason.
Lab services
e.g. Quest Diagnostics, Labcorp.
Direct-to-consumer panels
e.g. Function Health, InsideTracker.
Names are examples only, not endorsements. Your provider can often order the same test - and help interpret it.
